Membership applications and procedures (changes/cancellation)

To change registered information (for A, B, C members)

Type A (physician) members, Type B (non-physician) members, or Type C (facility) members falling under any of the following items should submit the Notification of Change form to the Society:

A change of work or home address

A change of address for delivery of Society materials (e.g. from workplace to home, or home to workplace)

A change in the facility name of a Type C (facility) member or Type S (supporting) member

Change of representative of a Type C (facility) member or Type S (supporting) member

Please submit the Notification of Change form via the Inquiry Form, FAX, or postal service to the Society Change and Membership Cancellation Section.

To cancel Society membership

To cancel Society membership, please submit the Notification of Cancellation form.
